As a new patient entering our office as a result of an auto accident there are a few things you need to bring with you and know before your first visit:
1) You will need to make a claim with your insurance company. Please bring the claim number with you.
2) If you have retained a lawyer, please bring their information with you to the appointment. We will need to call them to get information about your accident, insurance and the other party(s) involved.
3) You are required to get your own police report from the station. It will not be sent to you. Please bring the report with you to your appointment
4) Please inform us if you went to the Emergency Room or any other doctors after the accident occured. You will want to bring that information thus allowing us to fax them for the records the help us with your case. X-rays, MRI's and ER visits are essential information that will help us help you!
